New artist here, this is pixelated 3d.

I'm thinking he just shoves that shackle up her ass and pull something "important" out. Guts are meh.. Not sick enough for me.. Maybe if he pulls her spline out with skull still attached stretching her prolapse to the max. Any ideas?
