It does not have to have blood and mutilation to be sexy. I would love to see just a scenario with a man hiding in the bedroom closet and surprising two girls (who happen to be home alone) when they walk into the bedroom and break out their books to study. A nice strangle with a bit of nice, quiet struggling would be fantastic, leaving no marks on the bodies except for the angry red rings around their delicate necks of course. Then the dead, wide-eyed bodies could be stripped naked and savored...tasted everywhere and then completely enjoyed by their big killer. After he is completely done using both hot little bodies, he could leave the naked, dead girls suspended upside down by their ankles from the ceiling to dangle deliciously on full succulent display before leaving.
