Hi. I first made a part of the toes invisible using a picture for "cutout opacity" for the legs. I took a leg texture for G3 and painted a second layer over it in Gimp. What should be visible needs to be white, what should become invisible needs to be black. A caution: Using a picture map for cutout opacity can sometimes change the look of the skin of a character dramatically. And it will look strange in the viewport making it harder to pose characters.
Then I created a cylinder primitive. Then I painted a texture which is mostly red with a yellow circle symbolizing the bone and after a few tries managed to place it on the round front of the cylinder in the surfaces tab and make the sides just red.
The last step was making this cylinder small enough and then placing it into the hole left by making the tip of a toe invisible. Another caution: This sounds easy but navigating the cylinder to the right place was much harder than I thought. Also the fit to the toe can never be completely perfect because the cylinder is perfectly round while the toes are not. You have to choose a perspective where the inevitable clipping is not too visible.
The last step is parenting the cylinder to the appropriate part of the toe so it stays in place for later pictures if you move the character.
