Well actually I also wonder how painful is that for real, Because I think it is not really that bad as it looks like :) also if you are in aroused state full of adrenaline your pain tolerance is at least doubled.
I had some accidents like stepping on the nail and it was not that painful as it was scary not to get infected.
But in this story it is not the main goal to cause a much pain as possible rather than give everyone certain experience :)

I don't think heating horseshoes is common practice today as you are likely to burn your hands while handling that red hot horseshoe :) and imagine if she kicks and drops it on his lap LOL ;) better be safe :)
