I think a few pretty good settings for cannibal world stories are :
- Population imbalance. I've actually been thinking about doing some writing in a world around those lines, where some disease leads to few boys being born and high boys death rate, meaning that for humanity to survive, you need to have women have as many children as possible (so that enough boys get born) and to cull the excess population so as to be able to feed everyone. Then it makes sense to use those you cull as a supplementary food source to support the population, and to have a free use scenario to produce lots of births. I've been meaning to get the calculation done on what kind of death rates/populations would be needed to support such a world.

- Recreationnal cannibalism. In a world where killing isn't really taboo, maybe if there is overpopulation, so governments allow some level of murder to happen to keep population under control (maybe you could buy killing permits, for example), you could imagine a small portion of the population engaging in cannibalism, or in killing for entertainment/art. Of course, it wouldn't be so widespread as usually depicted in stories here, going so far as being in danger whenever you get on the street, or population would just drop, but it could exist to some levels.
- Still on the overpopulation theme, a world where there's not enough food to go around. People might resort to eating others or to sell family members in order to be able to get food, even if frowned upon.
