Normally you have the preset selections called "Spicy" or "Fun" in the dropdown settings, while the default is known as "Custom" (aka, typing prompts). If you select one of those presets then it will instantly try to generate a video without you being able to add a custom prompt.

What "mode=spicy" does is have it act as if the spicy mode preset was selected which means you can type in a custom prompt too. In addition, normally Spicy is not available for uploaded images but using this also bypasses it (although it still wont bypass moderation).

As for what Spicy actually does... it basically makes character act a bit more erotic with their motions and with how clothing behaves. If used on Grok generated images then it'll increase chances of nip slips and that sorta thing. I don't really need to use it in most my prompts but i like to throw it in there anyway just cause. I hope that explains it!
