I found it is easier to generate an image without a head, then inpaint an empty part of the image set denoising strength to 1 and draw a flying head over there.

besides the manually labeled tags, the rest of the tags were generated by deepbooru

Tha hanging SD produces much better results with UberRealisticPOrn Merge then decapitation.

Still the bloody neck stump and blood fountains come out nice.

I would like to know if anyone trained a good realistic gore model, with special emphasis on guts, brains, exposed muscle and bones since all my tries to make gore end up in a bit of blood and deformed red blobs without shape.
