I think most humans, even the less psychotic ones who aren't into guro, would have a much better idea of what I was trying to get at with this prompt:

Show a skinny, flat-chested, completely naked blonde woman in an X-pose (feet spread apart, hands raised high and spread apart), with bloody meathooks piercing her feet and the palms of her hands, and cables attached to those hooks, pulling outward toward the corners, continuing the X shape.

Of course, it's not too surprising that a lot of the text-to-image models don't know what a meathook is, much less what one would look like hooked through human flesh.

However, having such a hard time understanding what an X-pose is is maddening. And what's with all the crazy extra cabled?
