The new dildo tool in the dev version is doing something similar, you can just
replace its image with a knife (and tweak the code, so it does damage instead of pleasure)

The resolution is difficult to change and calculate if the ui has to occupy extra space, so I decided on this overlay. As a QoL it now cycles between right-hidden-left sides, so you can have easier access to the rightmost girl. (dev version)

Mixing classical animation and physics is always tricky, I'll try adding some fake force impulses in the future to certain joints to simulate convulsions.
