At first I though its easy, just override changeHP and call changePleasure from it, but pain and damage are quite different (e.g. getting shot in the heart deals a lot of damage, but isn't as painful as getting your hand skinned, which isn't lethal in return). In the future these should be distinguished imho, tho I'm not sure how other games handled this.
