I think I understand you. You mean that we shoudln't just identify with a fetish but that there are many different aspects of personality which sometimes correlate in weird, maybe opposing ways and that we shouldn't judge ourselves and others by just one aspect like getting a boner when people suffer.

Exactly. I like it because it makes me feel sorry for the victims.
That's why there's probably no final solution to that inner conflict. The contrasting urges(wanting to help and wanting to let them suffer) are codependent in the sense that they become stronger and weaker together. One will never overcome the other because they are a direct result of each other.
