There is no question which is true and which is fake self, true self is one which feels emotions. so it impossible not to know which is real.

so if you look into naked prepubescent and get aroused that means you are pedophile while your fake self may be someone who thinks that this is wrong. but not because it feels wrong but because it believes that it is wrong.
same is valid for gay who believes that being gay is wrong.

Problem of narcissist is that only true self can give pleasure and feel emotions. If narcist completely represses his true self he is doomed to live miserable life without other pleasure than pride and revenge.

If you worship (if that word is even usable here) your true self you are not narcist, only if you worship your image you are narcissist.
Also this grandiosity thing is not so much about inflated self-image but about importance of that self-image. it is mostly belief that what you do has importance in the great scale. while focusing on your true self, means that screw great scale, I only care to feel good and everything else may burn in hell.

But for practical purposes society exists and it can treated like multicellular organism with certain behavior. Behavior of that organism is totally different from behavior of individual people and people have little controll over behavior of that organism
