Again if all of that looks so horrible why do you find so much fun in blowing up your friends in the multiplayer FPS game? don't you feel any empathy for your friend whose body parts just got flying all-around when you shot him with a rocket launcher? Don't you feel any empathy when filling him with lead from the machine gun? That's should definitely hurt.

also expanding from getting a papercut into guts being ripped out is a bit far-fetched and if you do not have traumatic memory on the emotional level it is not going to generate an automatic response. You know that papercut or injection is just a very minor discomfort so even if you extrapolate from that and consider it to be slightly higher discomfort but unless you experienced that pain yourself you will never be able to understand it for real in form of emotional response.

Finally, it does not necessarily have to be real brutalization at all. A child (and maybe even an adult) will not object to a roleplay where he/he is being cut with a feather (or finger) or being cannibalized by someone pretending to eat them.
In fact, even speaking about brutalization according to Jordan Peterson children love that too of done in playfully form without damage and only light pain. Children love fighting /wrestling and other relatively rough play (which is in fact nothing else than sex) and they get very aroused when they do that.


Another aspect of guro for adults is that some people generally are attracted to things they fear and use that as a form of desensitization training. It is well known that we usually want to relive our traumatic experiences and fix them, make them different this time. so even if you experienced some horrible situation you may be fantasizing about it but also making it different from reality.
