Well, for me personally sex scenes in porn stories put me asleep, because more often then not they're essentially just a list of sex poses the author could come up with multiplied by the number of characters in the scene, and I fail to see what's entertaining about a list. Guro is a little better, because it has more extreme things happening, but in the end the point of things happening in the story is to feel emotions about them, and the point extreme things happening is to feel extreme emotions. Simply put: don't just write a story as a list of events, instead make the reader feel something about those events.

What exact emotion that would be depends on the story and your preferences (and as for your audience's preferences - whoever shares your preferences _is_ your audience). What is it that _you_ enjoy in the story? Is it the victim's pain? Then don't just say that the victim is in pain, but make the reader feel the same enjoyment from it that you feel. Is it the killer's bloodthirst? They don't just say he stabbed the victim, make the reader feel this bloodthirst with him… and with you. Ultimately it's not really about the characters' emotions, but about _your_ emotions and making them the _reader's_ emotions, as these are those that actually exist.

For example, my story "Black Ice" goes on and on in a logical loop of "dying for fun isn't really fun because you can't experience that fun when you're dead, but then that only makes it all the more fun". But then in the end whose fun is it if the characters don't actually get to experience it? Ultimately, it's the author's and the readers' fun. After all, objectively those characters don't really exist in the first place and serve only as a way for the author and the reader to have fun. Characters' emotions are ultimately just a means of making your emotions into the readers' emotions.

So, supposedly the events of the story make you feel something that makes you want to write about them in the first place, right? So then make sure to not just tell the events, but make the reader feel about them the same way that you do. Like, if you just say "A stabbed B in a dark alley" that's just criminal chronicles, not a story. But why is it interesting that A stabbed B? What does it make you feel that made you want to write about it? Make the reader feel it too.
