Presence/absence of a urethra just for starters. Shaft thickness and head to shaft size ratio. Positioning/orientation, how it's roote, how it appears to join to the body. Colour, smoothness, foreskin (even a circumcised cock looks markedly different to an exposed clit as a result). Many, many differences.

A greatly enlarged clitoris is anatomically a clitpris, but when of sufficient size it can function like a penis for the sake of penetrative sex acts, although without ejaculation as they are still just like normal clits in all ways apart from being of unusual size, the women who possess them are still 100% female biologically speaking. Enlarged clitorises are present in real life although never of a size comparable to that of a male penis, in fiction we imagine women with clits so large they can be used to simulate intercpurse with other women, and its tradition to make the clots so large that most male genitalia is shorter and more slender by comparison, as well the female would have a far greater stimulation from being penetrated by them than she would from being penetrated by the penises of any male partners she may have had.
