I checked Pixiv, there's skirtless versions, still not super clear but it's a bit more obvious at least. If you look closely in the first image, it looks like the middle rope connects to an animal collar around her neck, which is what's choking her. The chain the weight hangs on looks like it's attached directly to either her anus or her guts, the second image looks like her lower intestine was partially yanked out.

It's quite a brutal way to go, the collar means she'll choke more slowly than on the noose, and being only partially disembowelled means there's a good chance she won't die from blood loss before she chokes to death. It's so cruel, I love it.
