You'll find an area just steps away from the locked door—literally a 10-second walk. On the floor, you'll notice the same phi symbol to help orient yourself. From there, look upward at the ceiling, where you'll see square shapes along the sides and corners of the room. Some of these squares are red—that's what you're looking for. Don't get too excited, though. Behind the door isn't some secret ending or game over trigger, just a unique consumable item unavailable elsewhere in the game. It's genuinely helpful if you haven't already completed your playthrough.

Thank you for sharing, aciddemon. I personally purchased the game, and for anyone who can afford it, I strongly encourage you to support the creator by buying it as well. This is the same developer behind Demonophobia, and you'd be surprised how much they struggled to release Dysthansia. The more support we provide, the greater the likelihood we'll see a DLC or new guro games from them.
However, I do understand not everyone can afford or is even capable of buying the type of games we enjoy. This message is simply for those who can. Also, buy it as soon as you can, before their holy corporates up in heaven (or what us common folk call top floor of a skyscraper) pressure Steam to remove the game. But you got to feel bad for them holy corporates, we're interrupting their joy of selling guns and tobacco and engaging in human trafficking
