You guessed correctly that if it only applies to a small group of characters it could be A (Contemporary setting) but if you've got an entire country or world with casual nudity, sex and death then it fits into B (Alternative universe) unless there is also some science fiction or magical reason for it to be that way. Technically I'd consider a lot of fanfiction to be B by default as well.
