Basically everyone are immortal, but very cautious with making new people who are truly free. Most people are servants; Artificially-grown, genetically-engineered and raised to find fulfillment in servitude doing many kinds of jobs, and to willingly die. That last part serves multiple purposes; It ensures they won't live long enough to grow out of their programming. It's less cruel than just killing them against their will. And through one-way telepathy(which also reinforces control), masters are both reassured of their servants' lack of suffering to the end and get to experience the excitements of youth and being snuffed over and over again. Possibly, that reassurance is the original reason snuff shows even became a thing. Their bodies may also be modified to make their heads stay alive much longer.
