More precisely, that last mention of orgasms doesn't feel like it's meant to tells us that those orgasm existed (it's a bit late by that point), but instead mentions them as if something the reader was already supposed to know from before. But since the entirety of the hanging scene up to that point doesn't tells us anything about it, it leaves the reader confused "orgasms? what orgasms?", after all the last one they were aware of was before the whole hanging.

Like think you have a fight scene and character A punches character B. Then there's a few paragraphs of B kicking A's ass back with no mention of any more A's punches. The reader visualizes how A is overwhelmed and can't insert any of their own punches in-between B's. But then in the next paragraph you would say A's continuous punches finally had an effect. See what I mean?
