Listen, as a fetish, guro is inherently unrealistic and weird. You literally, physically can't get off to your heart getting dicked, that's not realistic. But, if you want to exist in society, you also can't just go out and kill people to satisfy a fetish, that's not realistic either. A free use society would certainly run out of people and collapse… but like, none of this is realistic.

I'm not saying to swing around in the other direction and just write total nonsense, and frankly, nobody is making that assertion, trust me. But if the guro fetish is to exist, like, AT ALL, a level of fantasy WILL be present.

The discussion has really boils down to a debate over how much unrealistic and weird is too much unrealistic and weird. If the events of a guro piece are too crazy for you, if it breaks your suspension of disbelief and you can't get off to it, then it's not for you. But it's clearly someone's fetish, and I really don't think anybody in the guro community has any sort of high ground over anyone else in that regard.

If it's not for you, it's not for you, that's just how it is sometimes.
