3- The person asking to take out the inmate will need to bring this form filled up and signed by a council member.

In case of Hole inmate:
There are additional treatment conditions and requirements if the inmate taken out is serving as Hole. All previous treatment conditions and requirements still apply.

Additional Hole treatment conditions:
4- The Hole has to be able to fulfill her role as Hole to the best of her abilities.
5- The Hole needs to wear the equipment provided by the prison of Stillwater at all time until said Hole is back to her duties.”
