the height of humiliation is being able to make someone so utterly ashamed about some innocuous, inherent property of their being that they would be willing to kill themselves to escape the feeling. what if instead of merely being humiliated for being a tranny, they were humiliated into being a tranny as well. humiliate a trap/femboy into mutilating their genitals, and then humiliate them even further for whatever botched axe-wound they come out with on the other side. doesn't have to be realistic or sane as long as it isn't immersion breaking, and as long as it meets the demand. creativity can find something that is worth putting effort into, i'm sure.
