The girl depicted being torn to pieces is Yuuka Tachibana, the female protagonist of the game. A group of powerful demons enter in the shelter where the main protagonist, Ayato Katsugari, and Yuuka lived in order to seize for "a feast" in the words of the demon Lamashtu who, alongside Asterah, Abbadon, Leraje, Decarabia, Iphos and Astarte, tore Yuuka apart and devour her body, leaving only her head behind as they leave the shelter.

Was this act done just because of wicked demonic malice? No. Yuuka is the human incarnation of the goddess Ishtar and by eating her body the 8 demons gain a fraction of Ishtar's power. This event just after the beginning of the game is what kicks in the whole story that involves humans, angels, demons and gods fighting for control over a Tokyo destroyed by ICBMs. Compared to all the other Megami Tensei games, Giten is by far the most obscure one since it depicts guro without ant censorship since it was released for PC compared to the other games that have been released for consoles.
